We also work closely with our colleagues based in the Primary Care Networks.
We have a 12 month secondment opportunity for an experienced Mental Health Nurse or Dual Trained Mental Health and Adult General Nurse to join our team in the Health and Wellbeing Clinic. This is a city-wide service based within College Keep, that provides a service to patients with severe or enduring mental health needs who are under the three Community Mental Health Teams in Southampton: Central, East, and West.
Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT.
Main duties of the job
To work as part of the Health and Wellbeing Clinic which is a city-wide service based with the Community Mental Health Team in Central Southampton.
To provide interventions such as depot administration, phlebotomy, and physical health checks according to the service users care plan and following local and national guidelines for the monitoring of medication and the physical health of the individuals under secondary mental health services.
To assess and monitor the service users response to prescribed medications, including the monitoring of Clozapine and high dose anti-psychotic treatment, escalating concerns as required and provide accurate information regarding medication advice.
To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.
To promote health and wellbeing including smoking cessation, attendance for physical health checks, and encourage healthy activities.
To liaise closely with GP Practices, pharmacies, and hospitals across the city to ensure effective communication and high-quality care of our service users.
To provide clinical supervision and caseload management to a small team of practitioners.
To identify, implement and evaluate, with others, areas for potential service improvement and agree further action. This may include participation in audit and service improvement.
