Job summary
Our friendly and dynamic Older Person's Community Mental Health Team are looking for a talented and motivated Mental Health Nurse to join us in delivering high quality care to our service users and families.
We aspire to provide excellent services, with person-centred care and recovery-focused interventions for our service users. We have an established, experienced and supportive team who you would work with and who can provide the necessary framework to sustain and develop your skills and interests. Our workload is varied and includes the Memory Assessment and In-Reach pathways.
We are looking for either an experienced Band 5 or Band 6 Mental Health Nurse who is keen to work with older adults with mental health needs in the community. They will be able to demonstrate an excellent knowledge of organic and functional mental illness as well as having some knowledge of physical health problems. They will also be expected to contribute to the wider working of the team, which would be participating in the duty system and supervising more junior staff.

Main duties of the job
To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.
To participate within the multi-disciplinary professional team in the relevant care group to deliver safe patient care.
To work in partnership with families and carers to ensure the patient's individual health needs are met.
Understand evidence based therapeutic interventions relevant to the care group to include education, clinical care, wellness recovery action plans and discharge planning.
