Main duties of the job
As a Band 5 Community Nurse you will work to keep people safe & well at home. You will work autonomously but closely with a wider team and use your clinical judgment to support evidence-based care planning for people living home, in care homes or other settings while making sure those who use our services have a positive experience. You will be able to develop skills, maintain & share your previous experience.
- Holistic patient assessment, formulation & reviewing care plans
- Management of pressure ulcers & wounds
- Palliative & end of life care
- Bladder & bowel care including catheterisation
- Medicines Management
- Injections
- Phlebotomy
- Provision of equipment to support care
- Safeguarding
- Line management & supervision of junior staff
- Practice supervision for nursing students
- Continual learning and professional development
Candidates must have a UK driving licence and use of a vehicle. You are required to work a range of early and late shifts on weekdays, weekends, and bank holidays. Our service runs 365 days a year from 8am-8pm.
