We are seeking an enthusiastic and experienced Band 6 Mental Health Practitioner, from a nursing or social work background, to join our team. Main duties of the job

Your key responsibilities will include -

Managing a Caseload

Manage a varied caseload, completing risk assessments, developing care plans, coordinating multiagency input, and building effective therapeutic relationships to support recovery and independence.

Providing Shared Care

Contribute to sharedcare reviews, support stepdown plans, and ensure smooth transitions between services through clear communication and sound clinical judgement.

Completing Assessments

Carry out comprehensive mentalhealth assessments, including risk, formulation, safety planning, and initial care planning, to guide safe and appropriate clinical decisions.

Supporting Colleagues and Students

Mentor students, support unregistered staff, contribute to teaching, and model reflective, evidencebased practice to promote a positive learning culture.

Working Independently

Deliver care in community settings, including home visits and clinics, using strong riskmanagement skills, effective planning, and confident autonomous practice.

Using IT and Records

Maintain accurate, timely clinical documentation; write clear reports; and communicate professionally using clinical systems and email.

Providing Holistic, RecoveryFocused Care

Work with GPs, social care, housing, crisis teams, and voluntary services to deliver coordinated, person centred, recovery focused support.

Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.
