Senior Community Physiotherapist - 22.5 hours per week
We are seeking an experienced Physiotherapist to join us. The North Hampshire Community Therapy Team is a friendly and supportive team of Community Therapists and Therapy assistants providing a domiciliary rehabilitation service to patients. You will develop your expertise treating a range of problems related to elderly care, falls, palliative care, orthopaedic, neurological and respiratory conditions. You will also have opportunity to develop your leadership and management skills within the team.
This post would suit an established B6 therapist or an experienced B5 with an interest in community working. Successful applicants will need to be an efficient worker, a good communicator and be confident working as an autonomous practitioner. Community experience is desirable. You will be responsible for managing your own caseload whilst working effectively within the multidisciplinary team to ensure an integrated approach to patient care - preventing unnecessary admission to hospital or long-term care, promoting functional independence and facilitating timely discharge from hospital.

Main duties of the job
*The Clinical Therapist will work across the caseload, using their clinical skills to identify the needs of patients, triage appropriately and liaise with the correct services.
*To provide specialist therapy assessment of patients. To provide appropriate patient centred treatment using evidence-based practice where-ever possible. Patients will present with acute or chronic conditions and complex multi-system pathologies e.g. neuro, respiratory conditions, orthopaedic rehabilitation, age related deterioration and palliative care.
*To provide the therapeutic treatment programme or where appropriate direct the intervention as necessary through Support Workers, or other members of the MDT.
*To facilitate safe discharge at the optimum point of care for patients, including assessing risk, ordering equipment and providing appropriate advice for patients/ carers.
*The Clinical Therapist will work under the supervision of the Clinical Team Lead receiving specific therapy clinical supervision/ development support from the appropriate leads in the district. The emphasis will be on developing assessment and clinical skills as well as developing skills that can be shared across a wider team.
*The Clinical Therapist will liaise with a wide range of clinical colleagues including but not exclusively, UCR team, District Nurses, Social Care professionals and GP's, to facilitate a patient focused, co-ordinated case management.
