Job summary

We are looking to recruit an enthusiastic and flexible Community Public Health Nurse 5-19 (SCHPN) (formerly School Nurse) who enjoys the challenge of working with children and young people.

You will work from a variety of settings including schools, partner agencies and homes working flexibly and creatively to best engage and meet the diverse health needs of the 5-19 population, as part of the Portsmouth Public Health Nursing 5-19 team based at Portsmouth civic centre.

You will lead the delivery of the Healthy Child Programme (DH 2009) within the locality and provide professional leadership to a skill mix team to enable the efficient and effective delivery of the commissioned service.

Main duties of the job

*To be the principal point of contact for schools within the area. To lead the development of co-operative working arrangements within these schools and their extended services; that support healthy outcomes for children.

*To be the point of contact for allocated GP practices; working in partnership to deliver integrated care that supports healthy outcomes for children and families.

*To identify and assess children and families in need and to work in partnership with Education, Primary Care, Children's Services, Voluntary Sector and other appropriate agencies, to deliver integrated programmes of care within the local area.

*To provide a continuum of support according to need, improving outcomes for children by reducing health inequalities.

*Work within Hampshire Safeguarding Children's Partnership and Southern Health NHS Foundation Trust (SHFT) policy and procedures, prioritising attendance at Case Conferences, reviews and planning meetings. To be responsible for the provision of case conference reports and court reports as required, contributing to safeguarding of all children. To support the Early Help agenda to ensure early intervention.

*Work within the framework of the Baby Friendly Initiative for Sustaining Breastfeeding in the Community.

*To promote and enhance health and wellbeing to children and their families through assessment, implementation of individualised health care plans and evaluation of care.

*To identify and assess children with complex health needs, offering an enhanced integrated service within the multi-agency arena, enhancing equality and opportunity for them.

*To be responsible for the quality and management of the RiO electronic patient record and data reporting system within the team.

*Promote and support an effective team ethos through the use of excellent communication skills, so that team objectives are understood and achieved

*Lead on induction for new staff and support students on placements within the service.

*Contribute to the development of SHFT clinical audits, policies, protocols and guidance for the 0-19 service and ensure that individuals within the team are aware of them.

*Take responsibility for own personal and professional development; maintain competence, knowledge and skills commensurate with role.

*Accountable for own actions and recognise own professional boundaries, seeking advice and support as necessary.

Provide effective leadership and management to staff which promotes high performance standards both individually and as a team, in the achievement of the Service and Trust's objectives and priorities.
