Main duties of the job
You will have a broad depth of mental health experience working with individuals who have a severe and enduring mental illness.
You will be responsible for care-coordinating a small caseload of service users with complex long term mental health problems who find it difficult to engage with services. You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health. You will work in a strengths-based and recovery-focussed way to support services users to meet their mental health needs.
You will have excellent communication skills to enable you to foster relationships and engage with a variety of individuals from a diverse range of cultural backgrounds and partner agencies.
You will contribute to weekly multidisciplinary team meetings and work in partnership with colleagues to evaluate care plans and harm-reduction strategies, adopting a 'whole-team' approach.
You will be a positive role-model, reflecting a commitment to maintaining a culture of learning from incidents and valuing two-way feedback.
You will be responsible for providing supervision arrangements, to ensure practitioners are offered opportunities to develop their skills, knowledge and keep up-to-date with current practice.
