Main duties of the job
To monitor the predicted and actual bed availability based on information provided by the Patient Flow Team colleagues and support them in effectively managing pathways, inpatient admissions and discharges for mental health services.
To work in close collaboration with the Patient Flow Matron and Patient Flow Clinical Manager and local teams to manage the capacity and demand for beds within Trust.
Monitoring patients remaining in place-based care, supporting admission allocation on urgency, risk and safety across the Trust.
Support monitoring of the patient in out of area placements, including cost, to ensure timely repatriation.
Attend all system calls.
To record, monitor, collate and communicate all performance data as required.
Monitor that al demand is gatekept and where this is not the case, this is escalated to the Patient Flow Clinical Manager or Patient Flow Matron
Collate and send daily reports as required
Line Management of Patient Flow Navigators
Support 8am-8pm, 7 day per week rota
Attend weekend system calls
