Job summary

Are you interested in supporting the safe flow of patients out of the hospital? We have Trusted Assessor Assistant role.

The post will focus primarily on working alongside the Trusted Assessors within the Transfer of Care Hub (ToCH) in a patient facing role. You will work collaboratively with the wider ToCH team and work within a Multi-Disciplinary Team (MDT) to help identify a safe and appropriate discharge for patients in the acute hospital wards.

Below are some of the activities you would engage in as part of the role.

To assist with reviewing patients on the acute wards

To work closely with the Trusted Assessors and the wider TOCH team to ensure all information we have about patients is complete and up to date

To review patients and their notes on the wards to make sure they are at the same level as when assessed

To assist the Trusted Assessors by going to the wards to see if the patients are fit to assess

To work with line manager if further information or communication with patients is needed

To work with MDT with information gathering about specific patients

Main duties of the job

To act as centralised point of contact for patients, receiving, triaging and responding to face to face, telephone and written queries, liaising and escalating to clinical team/ operational teams as required.

Provide explanations of procedure and processes to patients and their family regarding the pathway, verbally and by sending out relevant trust leaflets and instruction packs where appropriate.

Demonstrate high levels of empathy, sensitivity, calm and professionalism in all communications with patients and their families, recognising where referral to other sources of advice and support may be beneficial e.g., nursing or clinical colleagues.

Be aware of the potential for emotional, psychological and physical distress and symptoms that the patient may be experiencing, liaise and escalate to the specialist nurses or clinicians as appropriate.

Process confidential patient-sensitive documentation to and from patients, other healthcare professionals and external agencies.

Be aware of and take mitigating actions in circumstances where there may be a barrier to communication or comprehension, including coordination of best interest meeting.

Document and monitor all aspects of the care coordination and service delivery.

Ensure timely communication is maintained with the ward and the MDT and other care providers as required.

Assist people to access appropriate information and support by sign posting to a range of support services and encourage self-management where appropriate.

  • NVQ 3 or equivalent qualification or experience
  • GCSE English or equivalent
  • GCSE Mathematics or equivalent

Desirable

  • Training in E- Care logic, Paris, SystmOne Training (this needs to be within the first three months of being in post)

Experience

  • Advanced IT skills including Microsoft Excel and other software systems
  • Previous experience of working within a healthcare setting
  • Basic understanding of the patient journey
  • Self-motivated, and able to work independently as well as a team player.
  • Excellent administrative & organisational skills
  • Ability to work under pressure, multi-task and achieve tight deadlines
  • Ability to exert moderate physical effort for several short periods during a shift related to the movement of case notes
  • Ability to sit for a reasonable time and use a desk/laptop computer keyboard
  • Ability to prioritise work
