Main duties of the job

Support the Clinical Manager in day-to-day management of the active

community nursing caseload identified by community, primary and secondary

care colleagues, with the expectation that on average 75% of time will be

engaged in direct and in-direct clinical care.

Maintain clinical skills and expertise focusing clinical time on the holistic

assessment and review of complex cases to inform the development of a

comprehensive care plan and, wherever possible using available clinical time

to create opportunities to supervise, teach and assess junior staff in practice,

developing their competency in identifying subtle changes in condition and

knowing when to escalate within the team or to refer to specialist review.

Day to day ensure that the allocation of work is appropriate to staffing levels,

skills and competence of available staff and facilitate the prioritising of referrals

into the service ensuring that urgent referrals are responded to within an

appropriate timescale and feedback is provided if appropriate to the referring

health professional.

Undertake generic health and social care assessments using a holistic

assessment process and undertake specific comprehensive nursing

assessments for patients by using investigative and analytical skills for those

with acute/complex/ chronic or palliative care needs e.g. patients with multiple

pathology and complex social care needs.
