Job summary

The Long-Term Conditions Subdivision of the Community Specialist Division of Hampshire and Isle of Wight Healthcare NHS Foundation Trust delivers a wide range of clinical services across Portsmouth and Hampshire, providing personalised, integrated care that supports health and wellbeing for those patients in our care.

The Community Neurology Service, based in Portsmouth provides specialist neurological assessments and goal-focused treatment plans to enhance independence, support with the progression of their neurological condition and improve quality of life. Care is delivered in settings that best meet individual needs, including patients' homes, outpatient clinics, and community locations such as local gyms.

The Neuro Physiotherapy outpatient team, based at the Neuro Gym, Queen Alexandra Hospital, Portsmouth, treat patients from Portsmouth and some GP surgery's in Hampshire. They provide individual and group treatment which is based upon jointly agreed realistic and achievable goals between the patient and therapist in an outpatient setting

The post holder will be working alongside a multidisciplinary team that supports patients with conditions such as Multiple Sclerosis, Parkinson's disease, Motor Neurone Disease, traumatic brain injury, stroke, and other complex neurological disorders.

Main duties of the job

The Associate Practitioner will play an active role within the Community Neurology Service, delivering high-quality, efficient, and effective care to patients.

The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the guidance of the Physiotherapy Team. They will work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

The Associate Practitioner will demonstrate proficiency and competence, meeting the required qualifications outlined in the person specification. They will continue to develop through acquiring additional skills, knowledge and competencies in response to patient, service, and team needs.

The Associate Practitioner will be involved in the assessment and delivery of physiotherapy interventions for patients with a neurological condition, having the responsibility of delivering care to patients within a defined caseload without direct supervision. They will provide treatment to patients, monitoring patient progression, modifying and progressing treatment programmes as required and discharging patients or referring back to the registered physiotherapist when goals have been achieved.

The post holder will work predominantly within the Community Neuro Service (based from St Mary's Community Campus), flexibly supporting the Neuro Gym Outpatients as required.

Essential

  • *GCSE in Maths and English at grade A -- C/4-9 or alternatively Functional Skills in Maths and English at Level 2
  • *Hold one of the following qualifications: -
  • A Foundation Degree in Health and Social Care or a Physiotherapy-related field
  • or
  • A BSc in Physiotherapy / Occupational therapy
  • or
  • A level 4 or higher qualification in exercise provision with special populations
  • or
  • NVQ Level 3 (Diploma)
  • Note that some staff may hold the NVQ Level 4 Diploma (with the Level 4-5 Apprenticeship also relevant from 2018)

Experience

  • *Previous experience as an exercise technician / instructor or band 2 or above within physiotherapy or band 3 or above in nursing, occupational therapy or another paramedical role.
