Job summary

Based at Parklands House, Basingstoke, the In-Reach service offers assessment, diagnosis, treatment and support for people with a deterioration in cognition and/or dementia and over 65 who present with functional mental health illnesses who reside in a residential home in Mid and North Hampshire. We work closely with carers and other services, such as Adult Health and Social Care, GP's.

In-Reach is an acute service that aims at responding quickly to concerns and avoiding admission. As a support worker you will support the team in delivering the required care, support the care home staff with understanding and implementation of the care plans. We will ask for a high level of commitment, motivation for working with people with severe and enduring mental health problems and/or dementia.

Applicants must have a driving licence suitable for use in the United Kingdom and access to a vehicle for work purposes. Our working hours are Monday to Friday 9.00 to 17.00.

Main duties of the job

As a support worker you will support the team in delivering the required care, support the care home staff with understanding and implementation of the care plans. These can be around behavioural approaches to manage challenging behaviours and/or around monitoring medication efficacy. You will support the team by liaising with care homes, families and carers.

You will be required to accurately record service user information on electronic patient records.

You will work under supervision of a Band 6 health care practitioner, and will report back to the team.

You will be an active participant to meetings.
