Southampton West Older Persons Community Mental Health Team are on the lookout for committed and qualified professional to enhance their team based at the Western Community Hospital Millbrook. The Band 6 position presents a thrilling chance to conduct mental health assessments and offer advice and support to our clients.
Prior experience with the elderly and individuals with severe mental illnesses is preferred, whether in a hospital or community setting.
Expertise in Dementia care and assessment would also be an asset for the available position.
The role of a care coordinator is both challenging and fulfilling, serving as the initial point of contact for the elderly facing mental health challenges and significantly influencing the care we deliver in our community.

Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate.
Working under the supervision of Team Lead the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
