Job summary
We are seeking a Mental Health Nurse, Social Worker or Occupational Therapy team leader to join our developing primary care Mental Health Team in the PCN that covers the student population of Southampton. You will be based within GP surgeries and working with service users of adult working age including older persons with mild to moderate mental health difficulties. You will work within scope of practice so if have worked within specialities these skills will be transferable.
Main duties of the job
The role is clinical; you will contribute to the triage service; offering early assessment, brief interventions and signposting for people with mental health needs in their local communities. You will play an active part in the development of systems and processes which support care delivery within these teams. You will require good communication skills and be able to build positive partnerships with key stakeholders in primary and secondary care. You will work collaboratively with Solent Mind and have peer recovery workers work alongside practitioners in the surgery.
