Job summary

At Fareham Community Mental Health Team (CMHT) we are a dedicated team of professionals located in the heart of the population that we care for.

Our CMHT is lively, dynamic and consists of nurses, OT, Therapists, Support Workers and Psychiatrists. Our work is recovery focussed and partnership based. They team is proactive with a 'can do' attitude. We are an experienced team who feel passionate about providing evidence based care to patients and carers.

You will need excellent assessment and communication skills. Your role will be a caseload whose care you co-ordinate, assessments, duty, supervision, liaison with relevant partner agencies. As a senior clinician you will also write reports, quality improvement, safeguarding and run clinics as necessary.

You will need to have skills to manage the demands of the role, knowledge of MHA/MCA/CPA, and confident in delivering skills with the safety of our patients being at the forefront of your practice.

It is a supportive team however you will need to be able to work autonomously.

Car driver essential with use of a car during working hours. Usual working hours Monday to Friday 9-5

Main duties of the job

To provide psychiatric interventions to a defined caseload of patients and their families within Fareham catchment area. You will be care co ordinating your own caseload, covering duty, doing assessments, attend meetings, making appropriate referrals and liaising with partner agencies. the main part of your role will be to keep patients safe and provide a recovery focussed service by teaching them skills to maintain their mental health and enabling them to live a life they want. This will include risk assessment and safeguarding. you will be expected to keep up to date records and statistics of your caseload. good people skills and time management skills are key.
