Job summary
HIOW Healthcare are looking for an inspirational adult registered nurse to work as a Clinical Manager to be responsible for the management of Community Nurses as part of the Locality Health Team, with responsibility for ensuring safe staffing levels, optimum use of resources and continuity of high quality service delivery. The Clinical Manager will maintain a clinical caseload, whilst providing supervision and support for clinical staff day to day.
Community Nursing is much wider than the nursing team. We work alongside our colleagues within Social Care, our Internal Safeguarding Team, specialist services, both within the Trust and external providers, to ensure our patients are provided with the care they require. Working collaboratively enhances staff development but also ensures that our patients are at the heart of everything we do.
This is a fulltime role 37.5 hrs pw based at Oakley Road in Southampton. Our working patterns are across a 7 day week. Our core shifts are 0830 to 1630 and 1200 to 2000 hrs. There is also a requirement that you will work 1 in 4 weekends as well as bank holidays.

Main duties of the job
To be responsible for the operational management of a community team that are part of the Locality Health Team with responsibility for ensuring safe staffing levels, optimum use of resources and continuity of high-quality service delivery.
To maintain a clinical caseload but with a specific focus on supervision & support for clinical staff day to day, providing specialist clinical knowledge for complex cases & collaborative working with colleagues in primary, community secondary health & social care organisations.
Work as an accountable and autonomous senior clinician pro-actively treating, monitoring & anticipating changing needs of patients who have complex/and or chronic presentation to improve health outcomes & manage risk by increasing support enabling them to remain in their home/community & reducing avoidable hospital admissions.
Delivering on the organisational objectives to meet individual health care needs through high quality, safe & effective services to avoid unnecessary admissions to secondary care, support early discharge from hospital, effective case management for patients with complex long term conditions or following an acute health episode & individualised care for the patient & carers at the end of life.
The post-holder will be required to demonstrate a high degree of autonomy & professional accountability, working within the Nursing & Midwifery guidelines to have responsibility for the management of a defined Community Nursing caseload within 1 or more GP practices.
