Are you passionate about helping people regain independence? Join our ERS@Home team as a Care Assessor/Reviewer and play a vital role in delivering high-quality reablement care across Hampshire and the Isle of Wight. The team support people in the community to prevent admission to hospital as well as when they return home from hospital.
You'll assess and review care plans, collaborate with therapy teams, and support patients in their recovery journey. We're looking for someone with NVQ Level 2/3 (or equivalent), strong communication and IT skills, and a full UK driving licence.

Main duties of the job
Conduct assessments and reviews of re-enablement care
Communicate effectively with patients, families, and multidisciplinary teams
Maintain accurate and timely documentation
Identify and escalate clinical risks
Support service development and quality improvement
