Community Dietician

Job summary

We are recruiting for a Community Dietician to join our friendly team on the beautiful Isle of Wight.

You will be working as a member of the community and mental health dietetic team carrying out implementation, evaluation and delivery of the community dietetic service.

The successful candidate will work across an outpatient and community setting supporting the team provide a dietetic support to patients referred in for nutritional management.

Main duties of the job

To provide a complete dietetic service to designated community referrals and patients in outpatient clinics.

To provide a face-to-face service as well as embracing technology to carry our virtual clinics and patient support

Enhance the delivery of dietetic services to patients with diverse, complex and multi-system pathologies.

Coordinate and monitor the delivery of person-centred dietetic healthcare, using complex clinical decision making where appropriate. Ensure the practice is adapted to meet individual/ client needs/circumstances; considering cultural backgrounds

Educate patients, carers and healthcare professionals in the dietary management and prevention of disease.

Provide training to staff and patients on the importance of nutrition

To maintain accurate and comprehensive records of dietary advice given to patients and record clinical decisions in accordance with BDA and Trust/ legal standards, and to communicate treatment outcomes with other professionals as appropriate

To be professionally and legally responsible/ accountable for the management of the designated dietetic caseload and to organise this effectively with regards to clinical priority.

To be responsible for maintaining up to date knowledge of Adult and Children's Safeguarding procedures and attend professional meetings at the request of the safeguarding Teams /Social services.

Educate patients, carers and healthcare professionals in the dietary management and prevention of disease.

  • Effective user of computer software eg Microsoft and dietary analysis programmes
