Job summary
The advertised post is for a full time Trust doctor in Medicine for Older People and Rehabilitation. The post holder will be working for Hampshire and IOW Foundation Trust (HIOWFT). The Portsmouth and South East Hampshire (PSEH) division provides community care across PSEH geography City. This comprises inpatient and community-based rehabilitation services, community nursing and urgent response services, and specialist medical services. This post is to provide medical support for inpatient rehabilitation wards.
This new post will provide medical cover to our Spinnaker ward and to provide absence cover on Summerlee unit. The work involves admitting and completing medical assessments of patients as they are admitted to the ward, identifying and managing their medical problems, providing proactive care of chronic health conditions and responding to changes in their health. Enhanced medical cover will support the intention of taking more step-up admissions directly into the wards.
Main duties of the job
1. Clinical
1.1 Spinnaker Unit
This is a sixteen bedded unit at St Marys Hospital in central Portsmouth receiving general rehabilitation patients predominantly from Queen Alexandra Hospital but with step-up capability for direct admissions from the community. The role of the Doctor is to oversee the rehabilitation process, conduct Comprehensive Geriatric Assessment and future planning conversations with appropriate patients, deal with acute medical problems as they arise and advise on discharge and follow up arrangements.
1.2 Summerlee Unit
This is a thirty bedded unit again receiving general rehabilitation patients from Queen Alexandra Hospital. The unit is commissioned to deliver rehabilitation services to 15 people and Discharge-to-assess care to 15 people. However, this is flexible depending on demands at the time.
2. Non-clinical
2.1 Education
- Undergraduate education, primarily of year 3 and year 5 students completing their attachment in Medicine for Older People, this occurs infrequently but is good opportunity for medical, nursing and therapy students to gain experience of rehabilitation ward and multidisciplinary working.
- Postgraduate education, you will receive access to educational events from within HIOWFT. Study leave will be provided to support Continuing Professional Development.
- MDT training, delivering a teaching programme for community nurses and therapists. Supporting staff undertaking advanced history taking and examination skills, or prescribing courses.
