About the Role
The post holder, in collaboration with the Team, will work as part of Mental Health Services frontline response to people referred for support and treatment. The post holder will assess and support people presenting to services in a state of acute crisis.

Main duties of the job
As a 72 Hour Led Assessment and Discharge Facilitator, you will:
*Provide comprehensive assessments of needs and risks of people admitted to acute mental health services where risks have been assessed as not being outside of their scope of practice.
*Take responsibility for planning and the delivery of care to people requiring an assessment to identify acute needs and ongoing plans of care from mental health services, social care and third sector organisations.
*Be required to work into the Crisis Resolution Home Treatment Team.
*Plan and take responsibility in collaborating with service users and other services/agencies on how their needs will be met on discharge from this service.
*Participate in the development of the service including the ongoing development of this role.
*Ensure that relationships and own behaviour are managed in line with the Trust's Equalities Framework competencies.
*Ensure all communications are undertaken in a sensitive, confidential, and professional manner.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
