Job summary

Meadows is a busy community based mental health team supporting GP practices across Basingstoke and surrounding areas. We are looking to recruit a highly motivated, enthusiastic and experienced Clinical Team Leader The primary function of this role is to provide clinical leadership to the day to day operation of the team. To carry a complex caseload of service users and deliver specialist interventions, sometimes joint working complex cases with care coordinators. To provide clinical leadership to the day to day operation of the integrated clinical team, monitoring workload, allocating duties and ensuring duties are discharged safely and effectively. To provide expertise and guidance in complex and high risk cases, in line with Trust and Local Authority policies and procedures. To champion a culture based on principles of recovery, choice and personalised care, providing a role model to the rest of the team. To take a proactive approach to caseload supervision and management to ensure that practitioner skills are matched to clinical need, and that service users are able to flow through pathways of care. To deputise for colleague Team Leaders and CMHT Manager as required. To work as a Non-Medical Prescriber under the supervision of a Consultant Psychiatrist.

Main duties of the job

To be responsible for the day to day management of the clinical service, working closely with the CMHT Manager. To supervise and manage the daily workload in such a way to maximise efficiency, ensuring that duties are discharged safely and effectively. This includes the processing, evaluation and allocation of referrals as appropriate to the teams function, and an efficient caseload management system. To be available to practitioners for advice, guidance and support in the discharge of their duties, ensuring the team works within the care planning policy and risk policy. To ensure that care planning reflects agreed clinical care pathways, currently under development. To line manage an identified group of practitioners, including supervision, appraisal and development. To identify and address poor performance in line with organisational policy. To alert the CMHT Manager in a timely manner to any concerns about the teams effectiveness, or that of any of its members. To lead inter agency processes on behalf of the CMHT Manager in complex and or high risk cases, such as safeguarding, case conferences. To be involved in recruitment and selection. To act as an authorised signatory for employment changes. To work within agreed budgets NHS and LA. To deputise for the CMHT Manager as required, and lead on agreed areas of work for the team.

Essential

  • Relevant professional qualification. Mental Health Nurse with appropriate registration with NMC.
  • Management or leadership qualification and or training.
  • Experience of care coordination and the role and function of a Community Mental Health Team.
  • Completed training for Non Medical Prescribing.

Desirable

  • Evidence of other development and or training which indicates a commitment to continuous.
  • development and preparation for management.

Experience

Essential

  • Previous experience in the effective and positive management of complex and high risk cases. Ability to demonstrate a commitment to multidisciplinary team working. Must demonstrate significant post qualification practitioner experience. Demonstrate the ability to work collaboratively with other agencies. Willingness to demonstrate the potential for change management approaches which extend existing practices. Exposure to understanding of and competence in IT facilities and systems associated with service users, budgets and management information. Evidence of experience as a supervisor, including a clear understanding and application of an effective caseload management system. Demonstrate planning skills in terms of resource allocation and funding. Demonstrate an understanding of, and a commitment to the principles of recovery, choice and personalisation. Commitment to attend staff training and participate in own development through supervision and performance development.
