Job summary
An exciting opportunity has become available in our friendly and supportive team, Winchester City Community Team as a Community Charge Nurse. We are looking for an enthusiastic individual to fill our Band 6 role, to work in our mixed professional team in both patient homes and clinic settings.
Service aims include providing a local point of access, proactive case management and adoption of a co-produced approach with patients and service users.
The successful post holder will be required to provide expert clinical management for patients with complex care needs, management, support and leadership for the community nursing team supported by the Community Team Matron.
You will also be responsible for the recruitment, induction, clinical supervision and day to day/operational management of the Community Nursing Team and ensuring an effective contribution to the implementation of SHFT clinical, governance and other strategies.
The Winchester City team provides cover to a Winchester City and surrounding villages between the hours of 0800 am and 2000 7 days a week.
Main duties of the job
Monitor the quality-of-care planning, implementation and evaluation to ensure optimum quality is maintained.
Act as a role model in the promotion of person-centred practice, and challenge practice, which is not person-centred, so that a person-centred culture is maintained.
Ensure that discharge and transfer planning for patients is done in a proactive and interdisciplinary manner.
Practice, role model and promote safe and effective skills in all aspects of clinical practice.
Recognises own limitations in the provision of clinical care and urgency of patient's needs, referring to other healthcare professionals accordingly and is accountable for his/her own action.
Lead the training, support and supervision of the integrated service team, ensuring team training needs are met.
Support students in their learning and development.
Develop own professional skills and ensure these are shared with the team and wider division. Acting as a specialist resource.
Provide management responsibility for the team in partnership with the Community Team Matron (Band 7). This may include annual leave management; sickness' / absence management; performance management, training and development, appraisals, incident and complaints management.
Deputise for the Integrated Community Team Matron (Band 7) as required.
Ensure that all members of team receive yearly appraisal and development plans in partnership with the integrated senior clinical team lead.
