Support Worker ¿ Community Mental Health Team

Job summary

We are currently looking for a highly motivated Health Care Support Worker. Rivers CMHT provides compassionate, kind and recovery focused care to our patients, carers and their families. You will need to be able to work autonomously in the community and use your own initiative after you complete your training. The successful candidates will have good administration and IT skills and be able to type. You will also need excellent communication skills and you will need to work alongside other agencies to promote holistic recovery focused care. You will carry a caseload, take an active part in our multidisciplinary meetings and be part of the shared care team. You will have regular supervision and also have the opportunity to train and further your career development. You will also need to complete phlebotomy training as part of this job to enable you to work our clinics, alongside our psychiatric nurses.

Main duties of the job

To provide support to nurses in the depot and Clozapine Clinic, including phlebotomy. Work under the direction and supervision of qualified staff. To be able to undertake specific therapeutic roles with service users, that may include but is not limited to, Anxiety Management work, Graded exposure work, behavioural activation work and client-centred recovery focused care. To observe, report and record changes in the service user's physical health, mental health and behaviour. To give and receive clear messages by telephone and or personal contact. To identify and provide advice support to service users to help them complete daily living activities i.e. personal hygiene, diet and fluid intake, clothing and laundry, maintaining personal care/environment, sleep and activity, and physical observations. To act in a manner to respect customs, individuality, values, sexuality and spiritual beliefs, actively supporting the individual to fulfil these. To identify and provide social activities, emotional support and stability to service users to ensure effective engagement whilst maintaining professional boundaries. To contribute and be actively involved in the assessment, planning, implementation and evaluating of care. To provide escort duties to the service user under the direction of a qualified team member. To create and maintain professional supportive relationships with all members of staff, with other professionals and agencies to enhance recovery.
