Enhanced Wound Care Nurse

Job summary

An exciting opportunity has arisen within the New Milton and Lymington Community Care Team for an experienced Registered Nurse, primarily working in the New Milton Area as a Community Enhanced Wound Care Nurse.

You'll need 4 years post-qualification experience and a full driving licence with access to a car/motorised vehicle, as you'll be visiting patients in their homes.

You'll be expected to work weekends and late shifts, and complete additional training and competencies supported by a Tissue Viability Specialist Nurse, while developing your leadership and clinical skills in all areas required for a community nurse role.

You'll lead on wound care for the team, providing guidance, training and signing off skills and competencies. You'll also work independently with your own caseload, contribute to audits, and support wound care in a clinic environment, assisting the Clinic Enhanced Wound Care Nurse when needed.

Our team is part of an Integrated Community Care Team including Nursing, Physiotherapy, Occupational Health and Frailty services. Our service runs from 8:00am to 8:00pm, seven days a week, with shifts of 7.5 hours.

Main duties of the job

  • To work across a designated geographical area, supporting the delivery of evidence-based, high-quality specialist wound/skin care, with a focus on leg ulcers, and providing advice and education to staff.
  • In conjunction with a wide range of clinical colleagues, to lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people with complex tissue viability issues with a clinic focus.
  • To undertake a comprehensive competence programme to ensure high levels of knowledge and skill in tissue viability care across the team.
  • Primarily working in the community visiting patients in their homes, clinic settings, virtual clinics, and residential care to carry out tissue viability care and support general clinical staff in delivering care and competencies.
  • To act as a role model and expert in the prevention, treatment, and care of tissue viability conditions, supported by a Tissue Viability Nurse Specialist.
  • To participate in and influence efforts across health and social services to shape multidisciplinary pathways that support patient choice, improve quality of life, promote self-management, and enable early intervention through proactive care close to home.
  • To support the service in daily team operations including duty clinician, triage, allocation, delegation, and clinical visits in patients' homes. This includes working outside core hours, weekends, and bank holidays.
