Job summary

We have an exciting opportunity for a Band 5 Physiotherapist to join the South of England Rehabilitation Centre. A successful candidate would be expected to work as part of multi-professional healthcare team across two level 3 rehabilitation wards, with the potential to collaborate and support other rehabilitation wards on site, should the need arise. This would provide the successful candidate with a rich and wide exposure to complex rehabilitation patients with a multitude of diagnosis including (but not exhaustive): Orthopaedics (Trauma, Polytrauma and Elective), Non-acute neurological conditions (Multiple sclerosis, Parkinson's disease, CVA), Frailty and Falls, Surgical and Medical, as well as Spinal patients (via our Duke of Cornwall Spinal Pathway) or patients with admissions to ITU (via our Enrich Pathway). We also admit patients directly from the Community via our URS (Urgent Response Service) and CIS (Community Independent Service) pathways.

The purpose of this position is:

- To manage a caseload autonomously, with support from senior colleagues as required, undertaking assessment and treatment of patients within a hospital setting, with a range of diagnoses, and determine evidence based physiotherapy intervention, always with a view to facilitate discharge.

- Lead and supervise rehabilitation assistants, as well as participate in and contribute to team, training and service developments.

Main duties of the job

- Responsible for a designated caseload of patients requiring physiotherapy intervention, being able to, with support if required, undertake to organise and prioritise the required programmes of intervention, re-evaluating and modifying these as required. Seek support and advice with cases from more experienced colleagues when faced with matters outside of sphere of responsibility, knowledge or skill.

- Undertake specific physiotherapy assessments, including: Functional Assessments, musculo-skeletal and postural assessments, neurological and respiratory assessment and assessment of appropriate mobility aid and equipment. Use appropriate assessment tools and equipment to assist with accurate assessment and decision-making. Appropriately risk assess and document key risk areas.

- Plan, implement and develop rehabilitation programmes with clients, carers and ward staff through education and teaching with a view to encourage and empower clients and their carers to reach an optimum level of self-care and independence e.g. exercise programmes.

- Carry out safe moving and handling practice, with or without the use of equipment for patients within a hospital environment on a daily basis, in accordance with planned programmes of care and Trust policies.

- Communicate information to patients and carers e.g. about their condition, rehabilitation prospects and long term care needs, with support as necessary. This will include clients who have difficulties in understanding or communicating.
