Job summary
We are seeking an enthusiastic Consultant Diabetologist colleague to join our community diabetes services which have recently merged to form a single community provider across Hampshire. This is an exciting opportunity to contribute to a forward thinking, integrated model of care that places local care for patients at the heart of service delivery.
The successful candidate will join a team of six consultant diabetologists providing specialist diabetes care in the community to people living in Portsmouth, South East Hampshire, Southampton City, West and North Hampshire. The wider multidisciplinary team includes diabetes specialist nurses, dietitians, specialist registrars, FY2 and GP trainee doctors, trust grade and SAS doctors, lay educators, and administration staff.
Following the recent merger of community providers, the Trust is committed to reducing unwarranted variation in diabetes care across the region. This will require significant service development work, undertaken in collaboration with commissioners, primary care networks (PCNs), and other key stakeholders. The post-holder will be expected to contribute to this transformation agenda, helping to shape equitable, high-quality services that meet the needs of local populations.
Main duties of the job
While the successful candidate will be part of a Consultant team supporting care across Hampshire, they will hold defined responsibility for a geographical patch. It is anticipated that for this role it will include Portsmouth, South East Hampshire and Eastleigh. The Portsmouth and South East Hampshire area is an accelerator integrated neighbourhood team site, so collaboration with PCNs and the P3 team will be essential for the post holder. Close liaison with hospital-based colleagues will also be required to facilitate access to insulin pump and hybrid-closed loop therapy and for complication management including foot disease, renal and cardiology services.
The post-holder will provide specialist diabetes care, support the delivery of structured patient education, offer clinical advice and guidance, and contribute to the development of diabetes care in primary care settings. Clinics and education sessions are delivered face to face in community locations, with telephone and video consultations also available.
This is a 7 PA post based on 5.5 PAs DCC and 1.5 PAs SPA, although candidates wishing to work fewer PAs are most welcome to discuss their requirements with the department. Other interests (including research) may be able to be accommodated, however, and candidates are encouraged to discuss their preferences.
