Clinical Pharmacist - Community Inpatient Services

Job summary

An exciting part time Clinical Pharmacist opportunity has arisen on the Isle of Wight.

The successful applicant will work alongside senior therapists and nurses as well as GPs and Consultants and support two inpatient community rehabilitation wards at the Community Unit at St Mary's Hospital in Newport, IoW, The role will also provide a pharmacy service to the NHS funded beds at the Springfield Nursing Home in Shanklin.

We promote a supportive environment to enable colleagues to develop clinical skills. The post-holder will receive induction training and be mentored by experienced pharmacy colleagues. The team is forward-thinking, passionate about patient safety, looking to expand into community services, roll out EPMA, and increase numbers of independent prescribers.

Your work will focus on enhancing patient safety in medicines practice including medicines reconciliation, providing clinical professional advice to prescribers as part of a multi-disciplinary team, discharge planning, clinical audit, and teaching.

Key areas of practice:

  1. Utilise pharmaceutical expertise, working in partnership with patients, carers and healthcare professionals.
  2. Provide clinical pharmaceutical input to the physical health inpatient services in the Isle of Wight Division.
  3. Line manage the physical health inpatient services Pharmacy Technician.
  4. Monitor drug usage and promote cost effective prescribing within the Division

Main duties of the job

The role requires delivery of a high standard clinical pharmacy and medicine management service to designated physical health inpatient services, including attending relevant ward rounds, multidisciplinary team meetings and guiding doctors and independent prescribers in pharmaceutical matters.

The post holder will ensure safe and appropriate use of medicines in order to maximise benefit and minimise risk to patients, and undertake regular review of prescriptions, identification of significant drug interactions, advice for clinical monitoring, compliance with legal aspects of medicine supply and with the local formulary.

The post holder will have the ability to provide advice to health care professionals or patients who have been so far unable to access either information/advice, or require help where information is conflicting or difficult to interpret in the management of either individual patients, or groups of patients.

The post holder will have the ability to assess medicine regimens both during enquiry answering and/or routine visits to the physical health inpatient services providing highly specific advice to other clinical teams' members on the management of often complex medicine related issues.

This will require the willingness to take steps to ensure patients understand the purpose of their medicines and deal with any practical issues that may prevent the optimal use of their medicines.

In general, the post holder will demonstrate professional accountability to patients.

*To deliver a clinical pharmacy and medicine management service to designated physical health inpatient services, including attending relevant ward rounds, multidisciplinary team (MDT) meetings and guiding doctors and independent prescribers to ensure patients receive a high standard of pharmaceutical care.

* Facilitate the implementation of specified standards and policies, within current legal frameworks, regulations and best practice (i.e. safe and effective handling of medicines including controlled drugs) required to meet Care Quality Commission Essential Standards for Safety and Quality.

* Support the implementation of local and national recommendations, including local formularies, Medicines Management Group, NICE, MHRA and NHS England actions.

* Provide advice to ensure the safe and cost-effective use of medicines. Monitor medicines expenditure reviewing trends or by working closely with individual teams. Monitor drug expenditure feeding back results to relevant ward/team managers and clinicians.

* Act as an authorised witness for the disposal of controlled drugs in accordance with relevant legislation and Trust policy, regulations and guidance when required.

* To provide representation at relevant committees and groups where and when appropriate.

* To participate in training and supervision of undergraduate pharmacy students, trainee pharmacists, junior pharmacists, medicines management technicians and induction of new staff including nurses and doctors as required.
