Enhanced Mental Health Access Facilitator

Job summary

We are excited to be recruiting an Enhanced Mental Health Access Facilitator. You will be working alongside the Portsmouth Mental Health Hub, in the community to provide enhanced provision to the GP Practice and their patients.

The successful candidate will be enthusiastic and have a sound knowledge of mental health, to work in a creative and personalised way to provide advice, guidance and support in the management and treatment of primary care patients. The role will include sharing education and training to the GP practice staff and connecting to relevant support available in our city.

The Portsmouth Mental Health Hub supports early, easy access to support and advice to anyone over the age of 16, who has a concern about their mental health. It is a service which reaches into the community to ensure that anyone can access the right support at the right time. The service can make direct referrals into secondary mental health services as well as connect residents to community support.

You will be required to work flexibly across a 5-day week. Main duties of the job

[list=ul][li]Provide advice, guidance, and support to the GP Practices within a specified Primary Care Network on mental health needs to support the management and treatment of their patients.[/li][li]To work in creative and personalised ways to provide support, completing the work directly of value to the caller, ensuring their wishes and outcomes are always sought.[/li][li]To work with indirect supervision and show extensive use of initiative in prioritising and making decisions in relation to effectively supporting the smooth operation of the service.[/li][li]Work with the wider Access Team, wider mental health team and the PCN to identify new opportunities to enhance and strengthen the relationships and develop pathways between both teams.[/li][li]The opportunity for patients to follow a clear, helpful, and supportive process, where they would be screened and connected effectively to the most appropriate treatment or service related to mental health e.g., housing, financial and so on.[/li][li]Undertake person centred interviews over the telephone or through digital platforms which identify areas where the person wishes to see change and/or recovery and make an accurate assessment of risk to self and others under the supervision of a registrant within the Access hub or the PCN.[/li][li]Ensure that the Portsmouth Mental Health Hub understand the provision available through the PCN to best advise those contacting the service.[/li][/list]
