Job summary
We are looking for a motivated and enthusiastic Physiotherapist who has passion in helping support patients with the challenges in rehabilitation and can drive the team forward to excel in service provision. The post holder will be responsible for the clinical delivery across the Level 3 wards in the South of England Rehabilitation Centre (SERC).
We are an inpatients rehabilitation team based at the Western Community Hospital in Southampton (WCH). Our MDT comprises of Occupational Therapists, Physiotherapists, Rehabilitation Assistants, nurses, HCA, discharge coordinator, meds management team and Doctors. We provide rehabilitation for patient on 2 wards Keats and Conan Doyle 24 and 19 bedded respectively. Our rehabilitation focuses on getting patients back to functional level of independence and safe in the community on discharge, utilising standardised outcome measures to validate our productivity and prioritising patient centred rehabilitation goals.
At the WCH site in addition to Keats and Conan Doyle we have a Level 2b Neurological Rehabilitation ward and a Neuropsychiatric unit which provides a range of specialist rehabilitation and these wards together form the SERC.

Main duties of the job
Undertake skilled assessment using advanced clinical reasoning skills and a variety of assessment techniques in leading, planning, co-ordinating the delivery and evaluation of care.
To take a role in the specialist physiotherapy intervention to patients as part of the specialist team, assessing and treating own caseload of patients who may have a complex presentation and maintain associated records as an autonomous practitioner.
Undertake evidence-based projects, including recommendations for change in practice and participate in research and audit
Maintain a high-level clinical skill set within the team to ensure high quality and compassionate care for all patients.
Ensure all care delivered empowers patients to recover their independence at the earliest opportunity..
Lead at MDTs and PPMs and assist in complex discharge planning.
Independently assess patients mental capacity in line with the mental capacity act.
Demonstrate extensive knowledge in amputee, spinal, post ITU, polytrauma rehabilitation in addition to post falls, frailty, planned orthopaedic surgeries, End of life, and rehabilitation post acute admission.
