Community Nurse - Secondment

Job summary

An exciting secondment opportunity has become available as a Band 6 Community Nurse for Southern Parishes ICT. This is currently being offered on a fixed-term basis for 1 year. Note the closing date for applications will be the 24.08.2025.

The candidate should be a registered Band 5 Nurse with a proven track record of implementing change, being proactive, excellent leadership skills, be positive and willing to take on the challenges this role will entail.

The role will incorporate supporting the nursing team, providing strong holistic leadership which includes day to day leadership responsibilities and development of team members. The role will include influencing efforts across health and social services working in collaboration with other clinical colleagues such as GP's, Frailty Teams, Practice Nurse, and social care professionals. They will work across the caseload using their clinical skills to identify the needs of patients ensuring the most appropriate plan of care is provided.

Valuing teamwork is essential, and the successful candidate will be supported by myself and colleagues across the division to develop and excel in this opportunity.

Main duties of the job

Applicants must be highly clinically skilled. The position involves providing leadership support to the team on a day to day basis and to support the Integrated Community Lead as required.

Southern Parishes ICT is part of Southern Health NHS Foundation Trust and work as an Integrated Community Care Team including, Community Nursing, Occupational Therapy and Physiotherapy, Frailty Team.
