Extended Clinical Triage Practitioner

Job summary

We're looking for an experienced clinician to join our Frailty UCR/Hospital at Home Service as an Extended Clinical Triage Practitioner to take part in a pilot project. Our primary aim is to provide first-class healthcare in the home, allowing our service users the opportunity to remain at home and recover more quickly. You will perform a pivotal role, leading on triaging referrals coming into the Service which covers the Southwest of Hampshire and working closely with the Urgent Response and Hospital at Home Service covering Southampton city. You will ensure that referrers access the service with minimal delays and receive expert assessment, advice, and support to ensure patients receive timely, safe, and effective care.

What you'll do:

Lead clinical triage and prioritisation of referrals.

Ensure delivery of enhanced community-based care for adults who are experiencing a sudden decompensation in physical, psychological or social well being

Work collaboratively with primary care, ambulance services, and acute providers.

Monitor service capacity and allocate resources effectively.

What we're looking for:

Registered healthcare professional (NMC/HCPC) with relevant degree.

Advanced clinical skills (Level 7 in History Taking, Physical Examination, Diagnostics & Decision Making).

Strong leadership, communication, and organisational skills.

Desirable:

Teaching/mentorship qualification.

Independent prescribing qualification.

Main duties of the job

To provide a high standard of complex, enhanced care for patients, using enhanced levels of clinical judgement, skills and knowledge. Extended Clinical Practitioners will consult with patients, their family, carers and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans related to their field of expertise. They critically evaluate and analyse clinical problems using their expertise and clinical knowledge, seeking out and applying relevant evidence, enhanced clinical assessments, diagnostics, interventions, and equipment to make clinical decisions. Extended Clinical Practitioners deliver complex clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using critical analysis and their underpinning knowledge to manage complex interventions. They recognise and work within the boundaries of their practice, knowing when and who to refer patients to. To lead on incoming referrals to the service providing high standards of clinical triage/assessment & specialist frailty support and advice to referrers and other healthcare professionals.To support day-to-day incoming referrals in close consultation with the Frailty UCR/H@H Hub leads. Work collaboratively with primary care team, South Central Ambulance Service, and Acute providers. To monitor capacity within the service in real time to ensure appropriate referrals can be accepted.
