PCN Proactive Care Coordinator

Job summary

This Role is a PCN Additional Role and Responsibilities (ARRS) vacancy funded by the PCN to work for Southampton Central PCN as a Proactive Care Coordinator (PCN). Southampton Central Primary Care Network provides services across 5 Primary Care surgeries within the centre of Southampton including Solent GP Surgery, Burgess Road Surgery, Stoneham Lane Surgery, Alma Road Surgery and Homeless Healthcare. Holding a patient list of approximately 56,000 patients.

The key focus for the role is to lead supporting patients personalised care requirements. You will work closely with the PCN Proactive care team to manage an identified caseload of patients including individuals at risk of hospital admission, particularly those living with frailty, complex needs, or long-term conditions. Supporting those who require LTC review, and monitoring are on the relevant register, that their review appointments are in place, and working with the practice team and patients to ensure any follow up arrangements are in place.

You will work closely with all members of the practice and PCN team including the administrative team, nursing team, mental health team and PCN Personalised Care team.

You'll work without direct supervision showing extensive use of initiative in prioritising and making decision in relation to effectively supporting the smooth operation of the surgery.

Interviews with be held on Thursday the 6th November at the Royal South Hants Hospital, timings TBC

Main duties of the job

  • Establish and maintain effective communication system within areas of responsibilities, both internally and externally.
  • Liaise closely with all health and non-health professionals within the service line to identify any areas of administrative concern working towards resolution.
  • To effectively and frequently communicate complex sensitive and confidential information including patient data where required.
  • Regularly participate in working parties and to lead projects proposing policy change and service design to improve services and create efficiencies including departmental moves and relocation where required.

  • To be responsible for compiling statistics and reports as required.
  • To monitor and review all administration systems within the services to ensure efficient working practices to implement efficiency projects.
  • To liaise and communicate effectively with all levels of staff within the service and the wider Trust as well as with patients, carers, voluntary and external organisations.
  • To ensure data submission, analysis, activity reports, operational reports are up to date and users are aware of deadlines, planning, and prioritising workload appropriately.
  • To ensure appropriate risk assessments and other assessments are completed and documented in a timely manner in accordance with Trust policy.
  • To take responsibility for identifying own training and development needs, suggestions for improvements to the service and policy changes to the line manager.
