Job summary
As part of the Intensive Support Team (IST), the Band 5 Positive Behaviour Support (PBS) Worker provides direct, responsive, and person-centred support to the carers who support individuals with learning disabilities who present with behaviours that challenge.
The role involves applying PBS principles to assess, plan, support implementation, and evaluate personalised strategies that teach skills, reduce distress and improve quality of life. The post holder will work in line with the Trust's CARE values: Compassion, Accountability, Respect, and Excellence.
Main duties of the job
Support the IST Multi disciplinary team in completing relevant assessment & planning Positive Behaviour Support(PBS) intervention.
Assist in conducting functional behaviour assessments.
Contribute to the development of PBS plans tailored to individuals in crisis or at risk of placement breakdown.
Use trauma-informed and person-centred approaches to support individuals in achieving meaningful outcomes.
Provide training in individuals PBS plans.
Hold a small caseload of complex individuals with a diagnosis of learning disability who engage in challenging behaviour
Model PBS approaches to direct care staff and provide coaching and feedback to ensure consistent and effective implementation of support plans.
Participate in Periodic Service Reviews (PSRs) to evaluate the quality and effectiveness of PBS plans.
With supervision, collect and analyse behavioural data to inform ongoing support and risk management.
Contribute to dynamic risk assessments and crisis response planning.
Maintain accurate and timely clinical records.
Work collaboratively with families, carers, and multi-agency professionals.
Uphold safeguarding, Mental Capacity Act, and Deprivation of Liberty Safeguards principles.
Participate in clinical supervision, reflective practice, and team debriefs.
Engage in continuing professional development, including PBS and IST-specific training.
Support a culture of learning and improvement within the team.
