We are looking for someone to provide high-quality administrative support to the Frailty Team within the Primary Care Network (PCN), contributing to the delivery of proactive, coordinated care for patients living with frailty.
This role involves supporting multidisciplinary working, managing clinical and non-clinical data, coordinating meetings, liaising with patients and external partners, and ensuring smooth operational processes within the frailty pathway.
You will be a key point of contact for the team and help to ensure efficient day-to-day administrationThe role is to carry out office administration, working in support of the lead frailty nurse, acute visiting service and working alongside care coordinators to aid with supporting safe patient care. You will be required to use of a range of computer systems.As a first point of contact for patients, you will identify and prioritise information to inform decisions on the urgency of a response. The ability to work under own initiative and to prioritise own workload and to work to deadlines is essential.
Main duties of the job
- Provide comprehensive administrative support to the PCN Frailty Team, including scheduling meetings, managing calendars, and taking meeting minutes.
- Understand the importance of Key Performance indicators/service targets and keep the KPI data spreadsheet up-to-date.
- Accurately collate and record data into a central spreadsheet, ensuring it is kept up to date and ready for regular reporting.
- Support the coordination of multidisciplinary team (MDT) meetings, including agenda preparation, attendance tracking, and follow-up actions.
- Maintain accurate and up-to-date patient records and clinical documentation.
- Assist with the management of referrals, appointments, and patient communications.
- Assist in the monitoring and updating of frailty registers across practices, ensuring that patient information is kept accurate and current.
- Act as a first point of contact for patient and carer queries relating to frailty services, signposting appropriately.
- Liaise with GP practices, community services, social care, and voluntary sector partners to support smooth communication and care coordination.
- Support the Frailty Team in communicating effectively with patients, ensuring a compassionate and professional approach.
- Contribute to the ongoing development and improvement of frailty-related services within the PCN.
- Support implementation of national and local initiatives relating to frailty care.
- Participate in training and development opportunities relevant to the role.
