Job summary
0-5 ChatHealth is a text messaging service set up to support parents, carers and families of under 5's in Hampshire.
The service is run by Health Visitors and Community Nursery Nurses who have a wealth of experience of working with mums and dads-to-be, babies, toddlers and young children.
The 0-5 ChatHealth Service can support with advice and questions from parents, carers and families relating to a wide range of health and wellbeing issues including crying babies, infant feeding, starting baby on solid foods, sleep and behaviour problems. As well as giving advice, the team can signpost to appropriate services and other support.
Main duties of the job
*To lead delivery of the Healthy Child Programme including the 6 High Impact Areas within the community via the 0-5 ChatHealth text messaging service using evidence based practice within the four service offers
*To provide effective leadership and management to staff within the 0-5 ChatHealth Service, that promotes high performance standards both individually and as a team, in the achievement of the Trust's objectives
*To identify and assess any children and families in need, and to work in partnership with the Children and Family Services Teams, Primary Care, Children's Centres, Children's Services, Voluntary Sector and other appropriate agencies, to deliver integrated programmes of care within the local area
*Work within Hampshire Safeguarding Children's Board and Southern Health NHS Foundation Trust (SHFT) policy and procedures, prioritising attendance at Case Conferences, reviews and planning meetings. To be responsible for the provision of case conference reports and court reports as required, contributing to safeguarding of all children.
*To provide a continuum of support according to need, improving outcomes for children by reducing health inequalities. To support the Early Help agenda to ensure early intervention.
*To be aware of the Key Performance Indicators for the Children and Family Services and focus service delivery towards their achievement
*To be responsible for the quality and management of the RiO electronic patient record and other systems within the services

Job responsibilities
*To be a point of contact for GP practices and children's services within the county and to understand the integrated working arrangements with these services that support healthy outcomes for children
*To promote and enhance health and wellbeing to children and their families via the 0-5 ChatHealth Service through assessment, implementation of individualised health care plans and evaluation of care, referring into 0-19 Public Health Nursing Service where additional support is required
*To identify and assess children 0-4 with complex health needs, offering an opportunity for referral into an enhanced integrated service within the multi-agency arena
*To be responsible for the day to day delivery of the 0-5 ChatHealth Service to ensure the team delivers the Healthy Child Programme including the six high impact areas, in order to improve health outcomes for children and achieve SHFT key performance indicators
*To contribute to the development and implementation of effective clinical governance to ensure quality and manage risk within the team
*To review and develop the service ensuring staff and service user participation and feedback contribute to continuous improvement
*To be confident and competent in the use of digital platforms enabling families to access self help through both internal and external digital resources
*Escalating IT issues and network outage to the CDTL as a matter of urgency and enact the service business continuity plan
*To ensure timely communication with Health Visiting Teams and partner colleagues regarding referral and risk assessments to meet the needs of parents and those requiring further support
*Work within the framework of the Baby Friendly Initiative 7 Point Plan for Sustaining Breastfeeding in the Community
*To have safeguarding, management and clinical supervision at least every three months or as required as per Southern Health NHS Foundation Trust (SHFT) policies including annual appraisal and review
*To deputise for the CDTL when required to oversee the service
*To ensure that ChatHealth Service provided complies with the requirements of the licence, standards set out in CQC and in accordance with the commissioned services
*To ensure that incidents are reported on the Safeguard incident reporting system and actions taken as agreed by the CDTL
*Continuation of professional development by developing own knowledge and skills and those of others through learning and development opportunities
*To have knowledge of and comply with the lone working policy
*Work with the CDTL to ensure full coverage of the service is maintained and managed within hours of operation using the service E-Roster
*To work within NMC guidelines and to national and SHFT policies and procedures and within the Trust values
*Attend team meetings across the service to provide information, updates and data analytics in relation to the 0-5 ChatHealth Service
