Associate Practitioner - Complex Health Team

Job summary

An opportunity has arisen for an Associate Practitioner, within our Complex Health Team (CHT), in Portsmouth's Integrated Learning Disability Service - ILDS hosted by Portsmouth City Council. Our service has been rated as Outstanding in the last 2 CQC inspections.

As an Associate Practitioner you will work as part of the CHT in providing high standards of effective health care for people accessing outreach, enhanced supportive living services and or in receipt of Continuing Healthcare funding. Your role will be varied, working with service users, family, and carers that you will establish and maintain positive professional relationships with.

The successful applicant will provide high quality, efficient and effective specialist healthcare for adults with Learning Disabilities who have associated complex health needs, in partnership with other agencies and carers. This is achieved through a process of person centred care, which is comprehensive and responsive to the needs of the individual and their family carers. Identified needs will be assessed, treatments implemented and evaluated in partnership with service users as well as family carers.

As a member of the team, the post holder should possess strong interpersonal, communication and literacy skills, demonstrating an ability to work with a range of professionals, agencies and carers, and manage their own caseload. They will work under the direction and supervision of a registered practitioner.

Main duties of the job

To work as part of the CHT in providing high standards of effective health care for people accessing the outreach, enhanced supportive living services and or in receipt of Continuing Healthcare funding.

To act as a named worker to those allocated to you, as a member of the CHT, in leading and supporting the development of an inter-disciplinary approach to care, within both the supported living services and with outreach service users living in the community. Co-ordinating their care, facilitating the development, and delivery, of support plans, and annual reviews.

To monitor and review packages of care as appropriate in line with annual reviews, or as required.

To monitor and support complex healthcare packages that are responsive to changing health needs, e.g. around challenging behaviour, mental health needs and complex physical health. Promote philosophy of care in line with the organisational values.

Act as an advocate on behalf of the individual service users, and where possible, promote self-advocacy, within the boundaries of the professional role.

To plan and co-ordinate healthcare packages that are responsive to the individuals changing health needs, i.e. challenging behaviour, mental health needs and complex physical health. Including undertaking the appropriate specialist assessments and interventions.

Support the development of health action plans in line with Valuing People recommendations.
