The role will include visiting patients at home to assess for, and provide, therapy interventions in collaboration with the registered therapy colleagues within the team. The focus of the role is to support admission avoidance, to promote independence and to assist in the effective management of patients with long term conditions.
The post would suit someone interested in working within an integrated therapy team and wanting to achieve the best for their patients. They will work within a wider multi-professional team including Community Nurses and Advanced Clinical Practitioners to provide individualised patient care.

Main duties of the job
Work as part of the multi-disciplinary integrated Community Independence Service to undertake assessment, treatment and monitoring of planned care for patients who require rehabilitation in the community, seeking guidance from the registered professional as required.
Provide support and health promotion advice under the guidance of registered professionals.
May be required to supervise junior staff on a day-to-day basis.
