Job summary
HIOW Healthcare is looking for an inspirational RN to deliver a high standard of quality care to patients within the community setting. Community Nurses work autonomously and as part of a team and care is delivered in the patient's own home environment. Our nurses are responsible for a manageable caseload, working across the city ensuring our patients are receiving the best quality care from the comfort of their own homes and thereby avoiding hospital admission.
HIOW Healthcares Community Nurses work with adults who need support for a multitude of health conditions. These include:
- Long term conditions management
- Acute wounds and complex wound care
- Palliative and end of life care
- Facilitating discharge from hospital, including drain management, administration of prophylactic medication
- Continence care and bowel care
- Administration of a variety of medications
Although a lone worker, you will be supported, to allow you to develop your skills into becoming an autonomous practitioner in the community. You will be provided the training by attending appropriate training and working with our Educators in Practice and other staff.
This is a full time position working within a team that is based at Oakley Road, Southampton. Working patterns are across 7 days. Core shifts are 0830 to1630 & 1200 to 2000 hrs. There is also a requirement that you will work 1 in 4 weekends and bank holidays.
This vacancy is open to both newly qualified nurses and experienced nurses looking for employment within the trust.
Main duties of the job
A registered nurse working as part of a multi-disciplinary Locality Health Team affiliated to a cluster of GP Practices providing nursing care to the Practice population both in the patients home or clinic setting based on individual care needs.
Delivering on the organisational objectives to meet individual health care needs through high quality, safe and effective services to avoid unnecessary admissions to secondary care, support early discharge from hospital, effective case management for patients with complex long-term conditions or following an acute health episode and individualised care for the patient and carers at the end of life.
The post-holder will be required to demonstrate a high degree of autonomy and professional accountability, working within the Nursing and Midwifery guidelines to have responsibility for the management of a defined Community Nursing caseload within one or more GP practices in the absence of the Senior Community or Clinical Manager.
