Based at Parklands House Basingstoke, the Older Persons CMHT offers assessment, diagnosis, treatment and support to people with a deterioration in cognition and/or dementia and over 65's who present with functional mental health illnesses. We work closely with carers and other services, Adult Health, Social Care and with the voluntary sector, particularly Andover Mind.
CMHT is a lively, dynamic, supportive, multi-disciplinary team providing care coordination and recovery focused care for individuals with complex and challenging mental health needs.
You will need excellent assessment and communication skills and will carry a caseload and participate in the team's duty system. As a senior clinician, you will work alongside other agencies to promote holistic recovery focused care, produce reports, supervise staff and run clinics as necessary.
Successful candidates will have sound clinical skills, be able to coordinate complex care, have good administration/ IT/organisational skills, report writing and working knowledge of CPA processes and safeguarding.
Applicants must have a UK driving licence and access to a vehicle for work use Our working hours are Mon to Fri 9:00- 17:00.

Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate.
Working under the supervision of the Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
