Job summary

These are exciting and innovative posts to support the needs of the local population living with mild to moderate mental health difficulties. We are looking for registered Mental Health Nurses, Social Workers or Occupational Therapists to take a central role in the development of the mental health offer in two of our Primary Care Network's (PCNs) through the delivery of excellent, compassionate services to promote the recovery and well-being of patients, carers and families.

You will be joining a well-established, friendly, and committed team who prioritise patient centred practice.

The 2 part time roles (30 hours each) are based either in Winchester (The Clockhouse) or Basingstoke (Camrose Medical Partnership/Gillies Health centre/Beggarwood Surgery/Morland Surgery/Kingsclere Medical Practice)

Main duties of the job

As a Mental Health Practitioner you will work in a multi-agency way supporting the interface between PCN and Secondary Mental Health Services by providing specialist advice, support, assessment, treatment, education and solution-focused approaches to the local PCN teams.

You will be professionally accountable and responsible for patient care.

Triaging and undertaking initial assessment, signposting to appropriate services, or providing time-limited psycho-social therapeutic interventions directly, in GP surgeries and other primary care settings.

Providing clinical advice to GPs such as a specialist opinion, will contribute to the safe and effective day to day operation of the PCNs and the efficient and timely management of clinical enquiries and decision-making.

Maintaining clinical knowledge and skills by attending training and applying this in practice to support the development of the team.

You will take an active role in the developments of the Enhanced Primary Care Mental Health Team and with support deputise for the team leads in their absence.

Essential

  • Qualified as a registered mental health nurse social worker or occupational therapist
  • Mentorship course / evidence of CPD
  • Ability to fulfil travel requirements of the post
  • Clinical supervision training
  • More than 2 years' experience of working with clients with severe and enduring mental health needs

Desirable

  • Completion of in-depth safeguarding training
  • Additional qualifications in psychosocial interventions, substance misuse or brief interventions are desirable
  • Leadership training

Experience

  • Excellent assessment and risk assessment/crisis/contingency planning skills
  • Ability to triage referrals using clinical judgement skills
  • Awareness of common acute and long-term conditions seen in general practice
