Open only to applicants from Hampshire & Isle of Wight NHS system, which is limited to; Hampshire IOW Healthcare NHS Trust, Hampshire and Isle of Wight ICB, Portsmouth Hospitals University NHS Trust, Isle of Wight NHS Trust, University Hospital Southampton Foundation Trust, South Central Ambulance Service or Hampshire Hospitals Foundation Trust.
We are seeking a Mental Health Practitioner (MH Nurse, Occupational Therapist, or Social Worker) to join our established primary care Mental Health Team.
You will contribute to the triage service, offering early assessment, brief interventions and signposting for people with mental health needs in their local communities.
You will be a registered practitioner, experienced in working within a mental health setting with skills to engage people that may be pre-contemplative about change. You should be experienced in working with individuals presenting with mental health needs.

Main duties of the job
*To triage referrals using clinical judgement, offering assessment, formulation and brief interventions were indicated. Service commissioned for over 18 years of age.
*To support the planning and co-ordination of service users' care in primary care liaising with teams and agencies as required. To work on signposting to appropriate agencies for ongoing support.
*To discuss primary care referrals that require referral to secondary mental health care with the designated secondary care team
*To work closely with the secondary care mental health practitioner who is currently the link person for primary care services within secondary care for the Community Mental Health Team.
*To work with Primary Care stakeholders in the centre of Southampton to support and deliver therapeutic interventions to service users in primary care
