Job responsibilities
The post holder will lead on the provision of the Healthy Child Programme to children and young people outside of mainstream education and/or vulnerable group including young carers, those not in education & training, electively home educated, asylum seekers, homeless and gypsy, Roma, Travelling communities. The post holder will work with other practitioners to deliver care to those children and young people who experience social exclusion and multiple overlapping risk factors for poor health and development, such as poverty, abuse, and complex trauma. This definition is not one-size-fits all and is likely to evolve over time.
They will work with stakeholders across a number of settings and organisations to lead the service supported by a wider skill mixed team and working with a range of partners including education, children's services and voluntary sector.
-To use public health knowledge and expertise to lead delivery of HCP 5-19 and adopt an integrated service delivery approach for Inclusion Health Groups within Hampshire. This will include developing, planning, delivering and evaluating high quality, safe evidenced based care and interventions to the children, young people and their families falling within these groups and working collaboratively with other health professionals and partner agencies, including the voluntary sector.
-To work as an autonomous practitioner within a multi-agency arena, applying knowledge and expertise in health and wellbeing to enable children & young people, at risk of poor health outcomes, to reach their full potential (physically emotionally and socially).
-To work with partner agencies to identify and respond to the public health and wellbeing needs of children and young people within inclusion groups to determine the priorities for action. This includes attendance at partnership meetings and dissemination of information to service.
-Within the scope of professional practice and using appropriate frameworks, take lead responsibility to assess, plan, deliver and evaluate the health needs of specific groups of children to improve health and wellbeing.
-To develop, implement and evaluate health education and promotion sessions for parents and carers to support them to promote their child's health & wellbeing
-Effectively manage a caseload and where appropriate, delegate care to other suitably qualified practitioners and maintain professional accountability for supervising that care
-Work with the Area Managers and Clinical Team Leads to ensure operational delivery of the contracted services is responsive to changes in national and local policy with integrated care pathways delivered in partnership arrangements which support new models of care. The aim is to ensure that all children and young people in Hampshire receive safe, caring, effective, efficient, & timely access to The Healthy Child Programme.
-Ensure that data on health inclusion and outcomes of interventions is collected and used to inform local decision making.
-To provide support to the development of agreed quality standards for children and young people's health with a clear process for quality improvement, assurance and governance processes within the Division. This will include audit, and policy development in accordance with best practice.
They post holder will report to the Service Development Lead for 0-19 public health nursing team and be accountable to the Associate Director of Public Health for driving the child and young person inclusion agenda Hampshire wide.
They will provide professional leadership as a specialist practitioner to manage, develop and lead family-based care for those children and young people outside of mainstream education and/or vulnerable group including young carers, those not in education & training, electively home educated, asylum seekers, homeless. This will supplement the care given by the universal school nursing service Hampshire wide.
The role will have key specialist clinical, consultative, training, practice development and strategic responsibilities; the post holder will provide a visible, accessible and authoritative presence in the field of inclusion healthcare, acting as a professional role model by providing leadership and support within multidisciplinary, 5-19 public health nursing teams. They will provide expert clinical leadership with a visible, accessible and professional presence within agreed settings, working clinically alongside staff and be available to patients, their families and key partners offering information, advice and support on child and young person health where required.
They will actively engage with and involve service users in both care delivery and service co-design ensuring feedback is used to improve child and young people services for those identified within the vulnerable groups.
They will lead on the Healthy Child Programme to ensure all family members receive appropriate and evidence-based information how to improve children's health and wellbeing outcomes in a sensitive way using a variety of media.
They will work closely with Named Nurses for Safeguarding, who have responsibility for the protection of vulnerable infants and children and will be required to maintain up to date knowledge of local safeguarding procedures. Actively participate in safeguarding and clinical supervision to support the role in line with Trust guidelines.
Work with the Transformational Lead to support the established clinical supervision groups across the trust and contribute to a learning environment that promotes reflective practice and continuous learning in the field of health inclusion..
They will maintain professional registration / CPD through clinical competency and NMC revalidation programme. Develop and maintain up-to-date research-based specialist professional knowledge about child health. Develop and maintain a good working knowledge of child health legislation, policy and guidance. Have good up to date knowledge of local services available to those families who experience multiple risk factors for poor health and social exclusion.
They will ensure that the Children's division provide high quality, evidence based, joined up care for children, young people and families within the 0-19 Children and Families pathway. To support the development and delivery of quality assurance and clinical strategy to contribute to quality improvement within the Division. They will lead quality reviews within this specialist field and support the embedding of a robust quality and governance process that shares best practice and identifies areas for improvement and development.
Working with the Transformational Lead, Head of Nursing, Area Managers and Data Analyst team, they will ensure accurate quality data is used to provide assurance to the Children's Quality and Safety Board and Business Meeting in relation to key performance indicators around quality & safety of patient care for those families who meet the inclusion criteria.
To identify areas for audit and development relevant to inclusion groups; ensure audits are undertaken to improve outcomes for patients and ensure that recommendations are used to change practice.
Support and assure clinical effectiveness through the implementation of NICE guidance and guidelines by statutory and professional bodies as they relate to Inclusion Health Groups. Ensure that all Children's Division policies and guidance relating to Inclusion health groups are evidence based and appropriate. They will embed the principles of prevention, health and wellbeing within policy and support operational management of policy into practice.
Provide specialist advice to strategic groups and guidance on the policies and practices that impact on Inclusion Health Groups.
They will ensure that risks in the field of inclusion health are assessed and identified and will support the Children's Division to manage risks proactively, escalating where appropriate as per policy.
The post holder will work with the senior management team, Service Development Lead School Nursing, Practice Teacher, Community Practice Assessors, Clinical Team Leads to create a culture of learning that is based on a need for continuous improvement.
To provide clinical leadership support and professional direction within the skilled mixed team ensuring that high standards and safe levels of care are provided to families, children and young people. Ensure staff are competent and confident to deliver services to children and young people within inclusion groups; and ensure a clear understanding of the public health nursing within local Inclusion pathways referring to and working collaboratively with partner agencies as required.
To lead High Impact Area 4 (HIA) strategies working with School Nursing Area Manager and clinical team leaders and appropriate workforce to support the deliver of the wider integration of inclusion pathways.
They will establish networks and effective partnerships between school nurses, health visitors, midwives, GPs, mental health support teams (MHST), CAMHS and other agencies, for collaborative working that are focused on improving clinical quality standards for those children and young people within inclusion groups.
