Job summary
Join our dedicated Older Persons Mental Health Team as a Senior Community Mental Health Nurse. This role offers the opportunity to lead care coordination, conduct assessments, support duty rotations, and supervise others--all within a supportive, honest, and integrity-driven team.
Key Responsibilities
Coordinate and manage care for service users with complex mental health needs. Serve as Duty Worker on a rotational basis. Conduct initial and follow-up assessments (home visits and clinics) Provide supervision and mentorship to B5 nursing staff.

Essential Criteria
Full driving license and access to a vehicle for home visits and community meetings. Experience and confidence working with older adults and their families in community settings

Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to co-ordinate the care of a complex caseload of service users, supervising Band 5 Community Practitioners and Band 3 Community HCSW's where appropriate.
Working under the supervision of the Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their family and carer.
