Care Home Support Team Trusted Assessor

Job summary

Would you like to work as a Trusted Assessor within the Care Home Support Team working alongside the Transfer of Care Hub team ? You will be assessing patients within the Acute hospital identified as needing a residential care home placement.

The accountability within your role is in line with the Nursing and Midwifery Council or the Health and Care Professionals Council , which says that you must work together for the safeguarding and wellbeing of the public.

You will be acting as an advocate for patients to ensure the highest levels of care are delivered and will ensure you safeguard the delivery of high standards of care.

As a Trusted Assessor for the Care home support team you will be responsible for the clinical standards of care delivered within the service area.

Main duties of the job

Key Activities & Responsibilities.

To assess patients in the Acute hospital and to support flow from the Acute beds for those patients identified as requiring a residential care home placement.

To communicate work with all areas of the Care Home Support Team, Transfer of Care Hub and the multidisciplinary team to achieve the best outcome for patients.

Work alongside Care Homes to support with accepting patients into suitable placements.

To ensure actions are in line with the Nursing and Midwifery Council or the Health and Care Professionals Council codes and other statutory and organisational standards.

Educate and support staff. Identify staff development needs through the appraisal process.

As a senior member of the team, act as a role model to all members of staff, students and visitors.

Support the Care Home Support Team and TOCH to manage & monitor clinical standards in line with the Trust policies through audit process.

To provide information, including data, performance and notification of incidents and accidents as required.

Initiate the investigation of concerns related to poor clinical practice or staff management.

Escalate all concerns when there is a concern over the care being delivered by an individual to the appropriate service lead.

  • ENB 998 or other mentorship qualification; Minimum of 2 years relevant post basic clinical experience
