We are currently recruiting for a Community Clinical Lead within the Southwest Division.

The primary role of the Community Clinical Lead is to provide strong holistic leadership to the Community Nursing Team and have excellent clinical skills to support the wider team

Whilst this is an opportunity to develop further in your leadership role it is expected that you will provide excellent caseload management, advice and clinical support to patients in their own homes and in residential/nursing home care in the area. You will support the Area Matron and work alongside the other Clinical Leads in the division to strengthen the development within these services and enable local residents to be as independent with their own care as possible. The role will include day to day leadership responsibilities, providing clinical expertise and working closely with your peers in therapy and frailty teams. You will also work closely with other internal and external stakeholders.

You should have at least 5 years' community experience, two in a senior role with a proven community background. You will need to demonstrate strong evidence you are active in developing your leadership and management skills and show development in clinical skills as a Community/District Nurse.

Main duties of the job

Quality & Safety: Monitor and maintain high standards in care planning, medicine administration, discharge planning, and clinical procedures.

Team Collaboration: Build effective relationships with the multidisciplinary team, and communicate clearly with patients, carers, and colleagues.

Patient Focus: Act as a role model for person-centred practice, ensuring care is compassionate, safe, and tailored to individual needs (including palliative care).

Professional Practice: Stay updated on clinical developments, uphold professional standards, and document care accurately.

Leadership & Advocacy: Raise concerns that may affect care, promote a culture of wellness, and drive practice improvements.
