Continence Advisor

Job summary

We are looking for a Continence Advisor to work as part of the Specialist Continence Team to assist in the delivery of high standards of care to patients within their own homes, in clinics and in care homes. You will work under the supervision of the Team Lead and assist in promoting continence care and product provision, if required following evidence based practice.

In this role you will contribute to the delivery of all aspects of Continence Care for patients supporting clinics, community teams, nursing homes and reset homes.

Main duties of the job

*To plan, prioritise, organise and manage own workload, frequently working alone and acting independently or as part of a team.

*Ensure patient records are completed for each case, and that inputting is completed on a daily basis.

*Overcome barriers to understanding, e.g. patients' with hearing or cognitive impairment.

*Communicate with patients/carers using respect and empathy.

*Establish and maintain appropriate working relationships with colleagues, patients, carers and other healthcare professionals.

*Demonstrate dexterity and coordination when undertaking product assessments.

*Contribute to the requisitioning of product supplies for the patients as required.

*Work predominately independently on specified tasks, with regular support and supervision.

*Work with qualified practitioners with group work as required.

*Demonstrate an awareness and understanding of consent, and gain consent as appropriate following organisational policies.

*Share responsibility of indirect patient contact tasks, such as, answering telephones, arranging appointments and inputting activity data.

*Undertake health promotion education e.g. continence clinics, care homes.

*Undertake statutory and mandatory training as required.

*Partake in the Personal Development Review Process.

*Follow Trust policies and local procedures.

*Contribute to clinical audit as required.

*Complete staff survey as required.

*Undertake clinical supervision.

*Complete risk assessments and incident forms as required.
