Job summary

Hampshire and Isle of Wight NHS Foundation Trust has an exciting opportunity for an experienced Nurse to join a fast-paced supportive team based in a primary care environment. The successful candidate will conduct home visits to patients on behalf of the GP to assess and initiate treatment in their own homes or care homes. The successful candidate will work alongside our paramedic, physio and occupational therapists in our frailty team to provide proactive care management, future care planning and holistic assessments based on complex geriatric assessment.

We are looking for an experienced nurse with a commitment to providing compassionate care that responds to individual needs. The successful postholder will have experience in working autonomously, and as part of a wider multidisciplinary team, undertaking physical assessments for patients who are acutely unwell and need on the day assessment and treatment.

The post holder will be passionate about providing proactive care and support to prevent hospital admission and promote independence in the community environment.

The service is delivered from 8am-6.30 pm Monday to Friday. In this role you will work Monday, Wednesday and Friday, covering either 8-4 or 10.30-6.30 as per the needs of the service.

Main duties of the job

The Practice Nurse works independently using their own initiative and is accountable for their own professional actions. Provides support and leadership for other members of the Primary Health Care Team and leads towards the co-ordination of team activities.

*Conduct home visits to patients on behalf of the GP to assess and initiate treatment in their own homes or care homes.

*Work alongside paramedic, physio and occupational therapists in our frailty team to provide proactive care management, future care planning and holistic assessments based on complex geriatric assessment.

*Providing compassionate care that responds to individual needs.

*Work autonomously, and as part of a wider multidisciplinary team, undertaking physical assessments for patients who are acutely unwell and need on the day assessment and treatment.

*Provide proactive care and support to prevent hospital admission and promote independence in the community environment.

*Provide a holistic approach to individuals that includes analytical and judgement skills in triaging telephone calls to assess and evaluate the information given, use appropriate further questioning, diagnose a problem/illness and reach a clinical conclusion in accordance with agreed PGDs, guidelines and protocols.

  • *Current registration with NMC
  • *Educated to diploma level (Practice Nurse Qualifications or equivalent)
  • *Qualified practitioner / completed a minimum of one recognised level 6 Autonomous Practice Module relating to the diagnosis and management of either injuries or illness
  • *Up-to-date skills and experience in the full range of practice nursing services, including the nurse-led management of patients with chronic diseases.

Experience

  • *Post Registration experience
  • *Good IT skills -- ability to utilise appropriate electronic systems to accurately record data.
  • *Ability to work autonomously without supervision.
  • *Ability to meet deadlines and to work under pressure.
  • *Ability to adapt to the demands of a constantly changing environment.
  • *Aware of accountability of own role and other roles in a nurse led service.
  • *Knowledge of health promotion strategies and awareness of clinical governance issues within primary care.
