Job summary
An exciting opportunity has become available for Charge Nurse within our Andover and Winchester Rural North Team. We are looking for an enthusiastic individual to fill our Band 6 roll to provide strong holistic leadership to the team and support the Community Team Matron (Band 7). To include day to day leadership responsibilities, team development. Deliver creative, compassionate, and responsive leadership, enabling the team to achieve a well led culture.
The Charge Nurse will:
-Participate in the holistic assessment, implementation, and evaluation of programmes of care for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needs for example the physically disabled. Other settings include residential homes, nursing homes, clinics community centres and community hospitals.
-Participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management, and assure early intervention through the proactive provision of care
-Work across the caseload and the single point of access, utilising their clinical triage skills to identify the needs of patients, liaising with the correct services to ensure optimum care is delivered.
-Maintain clear and accurate records of care in line with standards required by the Trust and NMC.
Main duties of the job
Act at all times with Compassion, to be committed to delivering high quality Care, use effective Communication, be Competent and have Courage when needed.
Monitor the quality-of-care planning, implementation and evaluation to ensure optimum quality is maintained.
Inform the integrated senior clinical lead (Band 7) of any concerns with patients, relatives, visitors or staff that may compromise patient care.
Act as a role model in the promotion of person-centred practice, and challenge practice, which is not person-centred, so that a person-centred culture is maintained.
Demonstrate competence and confidence in clinical practice: this includes all clinical procedures that are relevant/specialist to the area.
Assess and maintain the skills and competencies of the team.
Support students in their learning and development.
Ensure new staff receive proper induction so that they understand acceptable practice and standards
Provide management responsibility for the team in partnership with the Community Team Matron (Band 7). This may include annual leave management; sickness' / absence management; performance management, training and development, appraisals, incident and complaints management.
Deputise for the Integrated Community Team Matron (Band 7) as required.
Ensure that all members of team receive yearly appraisal and development plans in partnership with the integrated senior clinical team lead
