Job summary
The Physical Health Nurse ensures patients with severe and enduring mental illness have access to physical health monitoring within the CMHT and to report findings back to the GP and CMHT as required. Management the administration and care of depot antipsychotic medications in depot clinic for patients with severe and enduring mental illness. Management of the phlebotomy and testing of bloods along with the care in the clozapine clinic. Ensuring patients in the community that may be housebound have access to physical health monitoring and their medication in an accessible way. Provision of expertise in their own discipline to the wider team and area. Liaison with the Registered Mental Nurses and Care Co-ordinators to ensure all patients in the clozapine and depot clinic receive regular risk assessments and the Physical Health Nurse will be an integral part of the annual medical review for each patient with the Responsible Clinician. Workingclosely with the registered Mental Health Nurses and Care co-ordinators to ensure all Key Performance Indicator targets are met for the patient group.

Main duties of the job
