Clinical Transformation Lead
Part-Time | Strategic Leadership Role
We are seeking a dynamic clinical leader to provide expert, visible, and flexible strategic leadership in the transformation of care across three key programmes: Healthy Ageing, Adult Mental Health, and Neighbourhood Health.
You will play a vital role in embedding clinical perspectives in service redesign, digital innovation, and system-wide change to improve outcomes, reduce inequalities, and enhance efficiency. You'll work closely with partners, networks, and those with lived and learned experience to ensure meaningful co-production at every stage.
Healthy Ageing, Older People & Frailty:
Focuses on transformation for people aged 65+, across physical and cognitive health. Key workstreams include proactive neighbourhood care, virtual frailty hospitals, RESPECT, and advanced care planning. Board meets Thursdays - 75% attendance essential.
Adult Mental Health:
Drives the shift from hospital to community care with assertive, trauma-informed services. Focus areas include complex needs, inpatient culture of care, and crisis/rehab services. Board meets Tuesdays - attendance essential.
Neighbourhood Health:
Shapes integrated neighbourhood models through system leadership and population health priorities. Emphasis on trust, collaboration, and shared vision. Board attendance essential.
Main duties of the job
We are seeking a dynamic clinical leader to drive transformation and improvement across our services. You will support the development of a clear vision for teams and stakeholders, providing expert clinical input to shape high-quality, sustainable care. Jointly accountable for the timely delivery of transformation programmes with measurable outcomes, you will champion innovation aligned to national and local priorities.
You will engage with key partners including Royal Colleges, Trusts, local authorities, VCSEs, ICBs, research and innovation networks, and those with lived and learnt experience. Promoting best practice, you'll ensure alignment between transformation initiatives and standards for safety, quality, and governance.
Working with divisions, you'll support implementation of new models of care, considering workforce, digital, estates, and pathway redesign. You'll ensure clinical engagement throughout, acting as a bridge between frontline teams and programme leads to ensure successful adoption.
You will monitor and evaluate the impact of transformation on clinical outcomes, patient experience, access, utilisation, and workforce. Co-production and meaningful engagement with service users and carers will be central to your approach in designing and refining services.
